Does your child have any special dietary requirements? * Agreements School Rules * I/We agree to abide by Ruapehu College rules and expectations as outlined in the Ruapehu College information booklet I agree Medical * I/We understand that Ruapehu College will take action on my behalf in case of sudden illness or injury and I agree to abide by College policies. I agree Computer Use Agreement * Access to school computers and the internet is by passwords. Sharing of passwords is not permitted. If another student is found using your password, it will be counted as an offence. Access to school computers is a privilege and not a right. Access is given on the understanding that the computers will be treated with respect. This means no tampering with, or vandalising computer equipment, no use of unauthorised software and no changing of system software settings or hacking. Use of internet. The internet is to be used for educational purposes. It is not to be used for: 1. Viewing or downloading of illicit, offensive, objectionable or illegal material. 2. Playing or downloading any type of game or music without the express permission of a teacher. 3. Downloading any images that are not related to school activities. 4. Accessing social networking sites such as Facebook, Instagram etc without the express permission of a teacher. Random checks will be carried out on the usage of the computers (including any portable storage devices used in school) to ascertain any inappropriate usage as outlined above. If any offences are discovered, then this will be dealt with according to the severity of the offence. Consequences could range from loss of access to computers for a period of time to police involvement for illegal activity and/or reparation (payment for) for hours spent repairing systems and hardware. Repeated breaches of this contract will be treated as continual disobedience and dealt with accordingly. I agree EOTC * Education Outside the Classroom (EOTC) This EOTC form is to gain parental permission for your child to participate in low risk activities and events in the Waimarino Rohe (National Park, Ohakune, Waiouru and Raetihi). These events will occur during the course of a school day and conclude prior to approximately 5.00pm. The activities will generally involve transport in school vans, school work, walking and interacting with community all under adult supervision. This blanket consent form will also cover common school activities outlined in the following list: - School Swimming Sports (Waiouru)- Mountain Biking Old Coach Road- School Cross Country (Ohakune)- Attending school related tangihana You will be notified through email, letter home or through facebook of your child’s involvement in activities covered by the scope of this form. Where an event involves risk exposure greater than what would typically be the case at school, such as adventurous activities or hazardous environments or the event continues overnight, specific consent will be required through the usual permission slip process. At the time of our seeking any further consents you will also be asked to update the health and contact information held by Ruapehu College. It is important that this form is completed at the start of each the year to cover students participating in the above EOTC activities. Details on this form will remain confidential to school staff, contractors and volunteers associated with supervising activities on EOTC events. Please note that is very important that student details such as health information and emergency contacts are kept up to date with the Ruapehu College office during the year. Privacy Statement: Please note: the personal information being collected on this form is for the purpose of running EOTC events. It won’t be used or disclosed for any other purpose except in accordance with the Privacy Act 2020. You have the right under that Act to access and seek correction of the information from the school. This information is held for the duration of the student’s enrolment at Ruapehu College. Updates are required only if there are changes to any of the information that is provided in this form. In an emergency school may act on my behalf School may administer pain relief I agree that if prescribed medication needs to be administered, a designated adult will be assigned to do this. I will ensure that prescribed medication is clearly labelled, securely fastened and handed to the designated adult with instructions on its administration. I will inform Ruapehu College as soon as possible of any changes in the medical or other circumstances. I am to be contacted if my child is to receive any emergency medical, dental, or surgical treatment, including anaesthetic or blood transfusion, as considered by the medical authorities present. If I cannot be reached in an emergency situation the school may act on my behalf. Any medical costs not covered by ACC or a community service card will be paid by me. To be read and signed by all participating students. I understand that any EOTC event is an opportunity for me to learn, practise skills and gain attitudes and values in an environment outside the classroom. · I realise that this requires me to take on genuine responsibility for my own learning and the safety and that of myself and others. I agree to do the following to make this happen: · At all times demonstrate the school values. Show courtesy and consideration for others; Follow the rules and instructions of staff and other supervisors at any event; Take part in all activities within challenge-by-choice options; Look after myself and my personal belongings; Declare medical conditions that could affect participation in the event; Accept the rules set by the school for any event, even if they are different from what is accepted at home. I understand that my parent/caregivers will be contacted and I may be sent home at their expense if: · My actions are considered unacceptable by staff; I break the school drugs and alcohol policy; My actions put me or others in any danger. I agree to my child taking part in EOTC events. I acknowledge the need for them to behave responsibly. I understand that there are risks associated with involvement in Ruapehu College EOTC events and that these risks cannot be completely eliminated. I understand Ruapehu College will identify any foreseeable risks or hazards and implement correct management procedures to eliminate or minimise those risks. I understand that my child will be involved in the development of safety procedures. I will do my best to ensure that my child follows these procedures. I acknowledge that in order to gain a better understanding of the risks involved I am able to ask any questions of Ruapehu College about the activities in which my child will be involved. I recognise that participation in such activities is voluntary and not mandatory. My child and I both understand that they may withdraw from the activity if they feel at risk. This must be done in consultation with the person in charge. I understand that Ruapehu College does not accept responsibility for loss or damage to personal property (either my child’s property or damage to other’s property caused by my child) and that it is my responsibility to check my own insurance policy. I agree School Transport Code of Conduct - For students who catch the bus to school This Code of conduct is between the student their caregiver, the bus operator and Ruapehu College. The caregiver and the student should ensure that they have read and understand this document, which is to be adhered to for the safety of the Bus Driver and all students travelling on the school bus. The Student agrees to abide by the behavioural expectations as described below: · When I am a passenger I will remain in my set for the whole journey. · I will not eat on the bus or throw anything inside or out of the bus. · I will not smoke, vape, drink alcohol, or take drugs on the bus. · I will respect other students and their property at all times. This includes pushing, verbal or physical abuse or any other behaviour which may distract the driver. · I will use socially acceptable language when conversing with the driver or other students and I will not speak at a volume that may distract the driver. · I will respect the property of the Bus Operator at all times, refraining from standing on seats or vandalising the vehicle in any way, including graffiti. · I will not engage in any behaviour that could put the driver or other students at risk. · I will observe the requirements and instructions of the Bus driver and the teacher responsible for bus duty at all times. · I understand that any damage I cause to the bus will result in the caregiver being billed for the cost of repairs. The safety and comfort of everyone on the bus depends on a standard of behaviour and consideration of others that is expected in the classroom. We expect parents and caregivers will support the school in maintaining these standards of behaviour. IF THIS CODE OF CONDUCT IS BROKEN · The student will not be allowed to travel on the bus for a period of one week and the caregiver will be required to find alternative transport to ensure that they are meeting their legal obligation to get their child to school. · After 3 offences the privilege of travelling on the bus will be withdrawn completely. I agree Chromebook Loan Agreement * In the event of a national lock down causing the college to close, Ruapehu College will loan a Chrome book and charger to allow students to continue their education from home. This agreement allows the college to act quickly and to distribute a Chrome book to those who have a signed agreement by their caregiver on file. Please read the agreement below. Note, the Chrome book will require an internet connection. If you do not have an internet connection at home, we will endeavour to provide you with copies of the work in a hard material format. Further, student work will be monitored by teachers via the Google Classroom platform. If the student is not engaged in the learning, a request to return the Chromebook will be made, hard copies of work given, and the Chromebook cleaned and reissued to any students on a waiting list. This agreement is between the college, the student and the caregiver. Terms and conditions of the agreement: 1. The Chromebook and charger remains at all times the property of Ruapehu College. 2. The Chromebook and charger MUST be returned when college reopens or upon request. 3. The Chromebook should be kept in a secured (locked) area when not in use. 4. Parents/caregivers are responsible to ensure that all due care is taken so that the Chromebook and charger are not damaged. 5. Loss, theft, or damage of the chromebook or charger must be reported to the college. 6. A home WiFi network is required to loan a Chromebook and charger from the college. 7. The Chromebook and charger is for the student’s use only and must not be loaned to, or used by, any other individual. 8. Web filtering is only provided when the Chromebook is used at school. Parents/caregivers are responsible for the monitoring to ensure online safety. I agree Digital or published Information. I/we consent to student photos and achievements being used for information or promotional purposes by Ruapehu College on school web sites, or in published material.
